Register online (This is the main registration route for all GP practices in England from 31.10.2024)

The QR code poster above is available in our waiting room for all patients to scan. You can also download and complete the paper registration form from the link below and drop this into our reception team if you cannot access the online registration option.

It is very important that we have your up-to-date phone numbers and address, so please let us know if you change any of your details.

Carers

If you are a carer for one of our patients, could you please inform the receptionist. Your name and phone number will be added to their record.

If you require more information about being a carer please telephone the carers’ helpline on 0114 272 8362. We also have a local Community Carers group who can be contacted on 07932 146441

Veterans 

Valley Medical Centre is proud to be an Armed Forces veteran friendly accredited GP practice.  
 
This means that, as part of the health commitments of the Armed Forces Covenant, we have a dedicated clinician who has a specialist knowledge of military related health conditions and veteran specific health services. This is important in helping ex-forces to get the best care and treatment.
