We welcome new patients from within our catchment area. This covers Stocksbridge, Deepcar, Bolsterstone, Midhopestones, Langsett and Thurgoland.
If you would like to register with us please come to reception or download and complete the registration form on this page.
We would also like you to complete the medical information questionnaire.
If you would like to register at our surgery please bring 2 forms of ID with you, including one with a home address. This can be a passport, driving licence or utility. If you have come from abroad please bring a copy of your passport and visa with right to residency.
You will be asked to book a new patient medical appointment with our health care assistants or practice nurses. This is to undertake a basic health check.
It is very important that we have your up-to-date phone numbers and address.

Carers
If you are a carer for one of our patients, could you please inform the receptionist. Your name and phone number will be added to their record.
