We welcome new patients from within our catchment area. This covers Stocksbridge, Deepcar, Bolsterstone, Midhopestones, Langsett, Wortley and Thurgoland. In some circumstances we can also accept patients living in Wharncliffe Side.
If you would like to register with us please come to reception or download and complete the registration form on this page.
We would also like you to complete the medical information questionnaire.
You will be asked to book a new patient medical appointment with our health care assistants or practice nurses. This is to undertake a basic health check.
It is very important that we have your up-to-date phone numbers and address.
