New Patients and Registration

We welcome new patients to the practice from the catchment area.  The practice covers the areas of Stocksbridge, Deepcar, Thurgoland, Langsett, Wortley, Bolsterstone and Midhopestones.

You will need to complete two forms when you register with us; one is the standard family doctor services form (GMS1) if you don’t have an NHS medical card.  The other is a basic health details form which provides us with information about any illnesses you have or medications you are taking.

Our reception staff will inform you about the forms you need to complete and give you a copy of our practice leaflet or, alternatively you can download them here.
